Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will have a strong background in behavioral health and social work, with experience in crisis intervention and management. This role involves working closely with clients to assess their needs, develop individualized care plans, and provide ongoing support to help them achieve their goals. The Case Manager will play a critical role in advocating for clients, connecting them with resources, and facilitating access to services.

Responsibilities

  • Conduct comprehensive assessments of clients' needs and strengths.
  • Develop, implement, and monitor individualized care plans.
  • Provide crisis intervention and management to clients in distress.
  • Offer substance use resources and support for individuals.
  • Utilize motivational interviewing techniques to encourage client engagement and progress.
  • Collaborate with multidisciplinary teams to ensure holistic care for clients.
  • Supervise and mentor junior staff or interns as needed.
  • Assist clients with navigating social services, including housing assistance and child welfare programs.
  • Facilitate access to healthcare services, including Medicare enrollment.
  • Support workforce development initiatives by helping clients gain employment skills.

Requirements

  • Bachelor's degree in Social Work, Psychology, or a related field; Master's degree preferred or some combination of experience and education that qualifies.
  • Proven experience in behavioral health and crisis management.
  • Strong skills in crisis intervention and substance use support services.
  • Familiarity with childcare practices and child welfare systems is a plus.
  • Experience supervising staff or mentoring individuals is desirable.
  • Knowledge of workforce development programs and resources.
  • Excellent communication skills, both verbal and written.
  • Ability to work collaboratively with diverse populations and multidisciplinary teams.

About Central Oregon Villages:The mission of Central Oregon Villages is to work as a community to safely shelter and empower our neighbors on their path to stable housing. Founded in 2021, Central Oregon Villages currently operates multiple emergency housing sites in Bend.

Our participants have chosen to live sober and drug-free and have committed to regular goal-setting and check-in meetings with their case manager.

About the Role

Case Managers build supportive, mentor-based relationships with participants who are working toward permanent housing. They report directly to the Executive Director, but also work closely with — and receive supervision from — the Director of Operations and Housing Navigation for any duties that intersect with operations, outreach, or housing navigation.

Essential ResponsibilitiesOrganizational Contributions

  • Ensure the safety and security of participants, staff, volunteers, and surrounding neighbors while providing emergency shelter.
  • Maintain teamwork and open communication with all staff to ensure program success.
  • Communicate with potential participants inquiring about shelter.
  • Perform thorough intakes and maintain accurate client records in HMIS and internal databases.
  • Attend staff meetings, program meetings, and case management meetings as scheduled.
  • Follow agency-wide policies, Standard Operating Procedures, and housing navigation processes.
  • Meet regularly with the Executive Director for case management supervision, and collaborate under the supervision of the Director of Operations and Housing Navigation when case management intersects with housing navigation, outreach, or operations.
  • Provide coverage and assistance for team members as needed.
  • Perform data entry and grant management duties as directed.

Client Services

  • Provide ongoing resource awareness, referrals, and individualized support for housing, food, clothing, education, vocational opportunities, financial guidance, health services, and advocacy.
  • Maintain professional boundaries and use trauma-informed, de-escalation approaches.
  • Conduct scheduled case management meetings with assigned participants, documenting meetings and goals both in our villages and with our program clients in the community.
  • Keep case management logs, action plans, and HMIS entries current.
  • Complete unit checks and verify completion of deep cleans.
  • Conduct urinalysis and breathalyzer tests during intake and as needed during stay.
  • Hold participants accountable according to program rules and regulations, responding with timely and thorough communication and documentation.
  • Assist in participant transitions to housing, including preparing documentation and coordinating under the supervision of the Director of Operations and Housing Navigation with housing navigation staff.
  • Maintain strong partnerships with outside agencies to coordinate referrals supporting ongoing housing stability.

Facility & Site Support

  • Collaborate under the supervision of the Director of Operations and Housing Navigation with operations staff to ensure participant exits are handled according to programmatic and facility protocols.
  • Report maintenance or safety issues promptly to operations staff.
  • Contact law enforcement or emergency services when required, and notify management.
  • Help maintain cleanliness in work areas and common spaces, modeling appropriate behavior for participants.

About You

  • Two (2) years of direct service experience or equivalent education, including case management with vulnerable populations, advocacy for low-income individuals, and/or experience working with culturally diverse groups and those with mental health and substance use challenges.
  • Knowledge of local service agencies for referrals and partnerships.
  • Ability to exercise sound, independent judgment, maintain confidentiality, and respond effectively to crisis situations.
  • Strong interpersonal and written communication skills.
  • Preferred: Certified Alcohol and Drug Counselor (CADC), Peer Support Specialist Certification (PSS), Community Health Worker (CHW), or similar.
  • Preferred: Experience with harm reduction, de-escalation techniques, and trauma informed care and training.

Position Reporting and (REDACTED) Compensation

  • Reports to: Executive Director
  • Works in close coordination with: Director of Operations and Housing Navigation (for operations, outreach, and housing navigation integration).
  • Status: Full Time, Exempt
  • Location: On-site(s) in Bend, OR
  • Benefits: PacificSource Navigator Silver 5500 (100% employee coverage), 11 federal holidays, PTO accrual

Job Type: Full-time

Pay: $23.49 - $28.29 per hour

Expected hours: 40 per week
